Registration Information
2008-09 Registration FOR SAINT CHRISTOPHER
SCHOOL
(In keeping with the School District of
Philadelphia, for kindergarten all children must be
5 years old by September 1, 2008; and for 1st grade
children must be 6 years old by September 1, 2008.
All children must be 4 years old by September 1,
2008, for our Pre-K.
We will be offering a half-day Pre-Kindergarten
program (8:00 to 11:30 am) and two full-day
Kindergarten classes (8:00 am to 3:00 pm). Both of
these programs have limited enrollment so we
encourage you to take advantage of these application
days.
Before we begin the process of registration, we
will check to see that all of the following
requirements are present. If not, we will ask you to
return with the missing item(s). The requirements
are as follows:
- be a registered member of Saint Christopher
Parish
- present your parish ID number
- know your child's Social Security number
- present an original, certified birth
certificate (for students entering Pre-K,
Kindergarten, and 1st grade)
- present a Baptismal certificate (if the
child was NOT baptized in Saint Christopher
Church)
- present dates and places of Penance,
Communion, and Confirmation, if applicable
- submit form no later than 8/25/08; each
Kindergarten or First Grade student applying
will be required to submit to the school the
signed doctor's form of a physical exam.
- present proof of immunization records for:
- Diphtheria- tetanus (4 doses- one dose
after fourth birthday)
- Polio (3 doses)
- Measles, Mumps, Rubella (2 doses)
- Hepatitis B (3 doses)
- Proof of chickenpox disease/vaccine
- Proof of TB test or risk assessment
- present copy of latest report card if
student is transferring from another school
(grades K-8). Interview may be required.
- present the nonrefundable registration fee
of $120 per family.
We are continuing our CARES program before and
after school. Your need for this service can be
expressed at the time of application. |
